Single Subject Credential

• Application form and fee
• Orientation interview (meet with program director for education plan)
• Three evaluations (see form below)
• Addendum - teacher education (see form below)
• Certificate of clearance (fingerprints)
• Verification of passing the CBEST
• CSET test results (at least one section passed) or an appropriately signed CSET waiver letter from a college or university with a CTC-approved teaching emphasis
• Verification of current negative TB test (last 12 months)
• Official transcripts of BA/BS degree (all institutions attended)
• Letter of explanation if your GPA is under 2.75

• Fingerprint packet
• Teacher education reference form
