Student-Initiated Forms
- Academic Petition – Request an exception to university policy, such as registering for or dropping classes after the deadline or extending an incomplete grade. Documentation required. View the full policy.
- Advanced Standing Request: Seminary – FPU undergraduate students may apply to have specific courses waive some FPU Biblical Seminary requirements.
- Certificate Application – For graduate students who have completed the requirements for a certificate.
- Course Challenge Exam – Use this form if you would like to test out of a course. A challenge exam fee will apply.
- Diploma Reprint Request Form – Request another copy of your diploma. If you did not receive your original diploma, please contact the Registrar’s Office at registrar@fresno.edu.
- FERPA Non-Disclosure – Request to withhold directory information.
- FERPA Release – Grant a person permission to review your academic records. To submit a FERPA release, go to Sunbird Central, click on the user icon in the top right corner and select “Student Records Release” from the drop-down menu.
- Foreign Language Test Proctor Request – Request to test out of the foreign language requirement(s). This form is used to schedule a foreign language exam.
- Incomplete Grade Request – Request an extension to complete coursework when circumstances dictate that you cannot finish a class on time. This form must be submitted before the last day of the class.
- Late Registration Approval – Register for a course after self-registration has closed. View the Registration Deadlines here: Registration Policy.
- Leave of Absence – Take a one-semester hiatus and remain active with the University.
- Major/Minor Change – Change your declared major or minor.
- Masters: Advancement to Candidacy Application – Submit the advancement to candidacy form the semester before beginning your thesis, seminar or final project. This provides an opportunity to check for completion of general degree requirements prior to the MA capstone course.
- Overload Request – For undergraduate students who want to enroll in more than 18 units in a semester.
- Registration Update – Add courses with conflicting meeting times or update grading information to Credit/No Credit or Audit.
- Student Information Update – Used by current students to update their student information. Use this form to update or correct your legal name, your social security number, birth date or marital status. You can report a change to your phone number or address in Sunbird Central. On the left, click "User Options," then "User Profile," and insert correct information. If you are not a current student, please email the Registrar’s Office at registrar@fresno.edu.
- Special Course Registration – Register for directed studies, independent studies, a graduate version of an undergraduate course or special units.
- Student Grade Change – Appeal the final grade you received in a class. Submit this form only if you have talked with your instructor first and cannot come to a resolution. View the full policy.
- Student Veteran Responsibility Agreement – Students using Department of Veteran Affairs-issued benefits, such as Post 9/11 GI Bill, will complete this form each semester to confirm the number of enrolled units to be submitted to the VA.
- Withdrawal from University – Withdraw from all classes and leave the University.
Department Chair Initiated Forms
- Edit or Create a Course Section – Use to request edits to an existing course section or the creation of a new course section.
Program Director Initiated Forms
- New Course – Use to request the creation of a new course in the catalog and system.
- Course Change – Use to change characteristics of an existing course (e.g. title, prerequisites, course description, etc.)
- New Program Information – Use to request a NEW University Program.
- Program Changes – Program Directors use to request and update to the current program.
Instructor Initiated Forms
- Instructor Grade Change – Use when a clerical or computation error has been made, or for updating a grade due to incomplete work.
Advisor Initiated Forms
- Course Waiver – Use to request a course be waived for a student.
- Course Substitution Request – Use to request substitution course for a student.
